Prep:
Save the cardboard box and place the gel pack flat into the freezer (for at least 4 hours) to ship the sample back. Send the sample Monday-Thursday only. Avoid apples, grapes, pears, and cranberries as well as their juices or products 48 hours before collection. Avoid arabinogalactan, reishi mushroom, echinacea, and ribose supplements for 48 hours before collection. Females: Do not collect samples during menstruation. Limit liquid intake starting at 6 PM the night prior to collection to avoid sample dilution. Urine must be yellow in color. If urine is clear, the sample is too diluted. Discard diluted sample, rinse the collection cup in hot water only (no soap), air dry, then re-collect.
Step 1:
Fill out the test requisition form (please print clearly). Print the patient’s full name, time and collection date on the side of the urine cup. Collect urine sample in yellow-cap plastic urine collection container. Urine must be yellow in color. If urine is clear, the sample is too diluted. Discard diluted sample, rinse the collection cup in hot water only (no soap), air dry, then re-collect.
Step 2:
Place urine specimen into the biohazard Ziploc bag along with the absorbent packing sheet. Place frozen gel pack and biohazard Ziploc bag (which contains the urine sample) into the silver thermos bag. Place the silver thermos bag into the box. Fold and place the completed test requisition form in the box and close the lid.
Step 3:
Place the box in the FedEx Clinical Pak. Note: The urine sample MUST be in the cardboard box or the shipment will be rejected by FedEx. Seal the FedEx Clinical Pak. Please keep the purple FedEx Billable Stamp (including tracking number) for your records. Ship out at your nearest FedEX location.
